MY Health Care & Aesthetics is pleased to offer immigration physical exam services. Dr Yang a licensed US Civil Surgeon who has been authorized by the US Immigration Service to conduct immigration physicals for the Department of Homeland Security. Individuals applying for citizenship or seeking Permanent Residency or a Green Card are required to obtain an immigration physical performed by a designated Civil Surgeon prior to approval of their application. The exam results will be recorded on Form I-693 for the applicant to return to the US Immigration Service. The exam process generally takes 3 to 5 business days, depending on tests or vaccinations required.

To schedule your initial medical exam you are required to pay a $100 deposit, all remaining expenses must be paid at the time you check in for your exam. If you cancel your appointment with 48 hours notice, we will refund your deposit, if you fail to do so, your deposit will be non-refundable.

What to bring to your medical exam?
  1. Valid, government issued picture ID such as a valid passport (preferred), driver’s license, green card, or military ID.
  2. Medical records (translated to English by an official translator, if necessary), including proof of prior treatment for tuberculosis or other infectious disease and prior chest x-ray reports.
  3. Immunization records (translated to English by an official translator, if necessary).
  4. Any mental health records (translated to English by an official translator, if necessary).
  5. An immigration physical Form I-693 (form version dated September 13, 2021), pre completed with your information IN CAPITAL LETTERS. Please make sure this form is the most current form available from the US Immigration Service.
  6. Please budget at least 1 to 2 hours for this process on the first day and a total of 1 to 4 business days depending on the necessity of additional testing (Friday exams may take longer for labs to process). Additional labs may be needed at an additional expense and may require up to 1 to 2 weeks for a result, depending on the test.
  7. If you do NOT speak fluent English or Chinese, you must bring a translator with you. The cost of your translator is at your expense .

Proof of vaccination will need to be provided to this office before your final paperwork can be forwarded to the US Immigration Service.

What happened after the exam?

Once the immigration medical examination is completed and all immunizations and/or lab tests have been performed, we will provide you with a certified Form I-693 in a sealed envelope to present to the Department of Homeland Security and US Immigration Service. DO NOT OPEN THE SEALED ENVELOPE. Turn in the sealed envelope with your immigration application. A copy of the blood tests results and exam will be provided to you to keep after results have been discussed with the doctor. The medical examination will only be valid up to a year after the immigration exam date. Our physician will not certify or sign your I-693 until all medical exam and additional testing fees have been paid. Fees are required to be paid at the time of service.

Typically, you will be notified of the results of your lab work (and chest x-ray, if done) within 7 to 10 days. If you have a health condition requiring medical follow-up, clearance will not be granted until follow-up is completed. You will be responsible for the cost of any additional medical examinations or treatments required to fulfill the federal health requirements of the US Immigration Service.

Will my insurance cover the cost of the immigration physical exam?
No. We ask you to pay in full at the time you check in for the exam.
How much does the immigration physical cost?
The cost of the basic immigration medical exam is around $450.00. This includes blood work (test for syphilis,TB, gonorrhea), physical exam, and follow up consultation to pick up results and certified forms. If you need a chest x-ray, blood titers for immunizations, or other testing there would be an additional charge. There will also be an additional charge if a confirmation syphilis test is needed and/or other testing as required by the US Immigration Service. All fees must be paid at your time of service.
How long will it take before the results and documents are ready?
Your forms will be ready in 3 to 7 business days depending on results from laboratory testing. Laboratory test results usually return within 2 to 5 business days (Monday thru Friday). If the tuberculosis test is positive, a chest x-ray is required, which can add another a few days to the process.  You will be required to return to our office to review your results and pick up your certified forms.
Can I get my chest x-ray and blood work done somewhere else and bring the results with me to my medical exam?
If you have had a chest x-ray within the past year, bring the results with you to your exam and we will decide on a case-by-case basis if the xray is acceptable. Results must be signed by the physician completing the test or interpretation.
Are immunizations included in the price for my immigration medical exam?
No. The cost of immunizations and their administration cost extra. We utilize nearby pharmacies to have your required vaccines administered
Is there a waiver available for applicants who cannot afford the required vaccinations?
No. The US Immigration Service determines which are required and there are no exceptions.
When receiving immunizations from a health department, does our physician have to sign Part 6 of the Form I-693, or can the health department immunization physician/staff sign it?
The signature in Part 6 must be the physician at the health department if the immunizations are administered there. It may be an original or stamped signature. The health department nurse or other health care professional may, but is not required to, co-sign the vaccination supplement. Part 6 will be completed for those applicants who are filing based on refugee status.

The mandatory requirement includes:

  • Quanti-Feron Gold TB Test or T-Spot Test

The following list of additional blood tests may be required, based on the patient’s age and documented vaccination history.
